QUESTION:
Dr. Ally, a 49-year-old professor, was diagnosed with essential hypertension 12 years ago and
was on antihypertensive drugs. However, he did not take his medications last year because he
was feeling just fine. In addition, he was very busy with work. Nevertheless, he felt tired after
work and developed dyspnea while climbing the stairs. Recently, he had a bout of epistaxis
(severe nose bleed) with dizziness and blurred vision. He went to the doctor for a checkup. His
blood pressure was 180/110, and the doctor found rales or crackles on his chest upon
auscultation. The doctor ordered rest and asked him to start his medication again.

FORMAT & EXPECTATIONS
Title Page The title page is separate from the rest of the report and must include the
following: week and number of the assignment, title of the assignment, South University
Online, Human Pathophysiology, instructors name, your name and the date submitted. In
addition, the name of the assignment (e.g. Diagnostic Techniques, Anemias,
Hypertension, etc.) must be in the header space in the left upper corner and the page
number in the right upper corner starting with the title page. See page 5 for an example.
Report Format The report should be a Word document, two-to-three pages long,
excluding the title and reference pages unless otherwise specified in the weekly
announcement. It should be double-spaced using Times New Roman font number 12 with
one-inch margins. Details of the APA format with templates and examples may be found
in the South University Library under Copyright and Citation. Answers to individual
questions should be in separate paragraphs. The questions may head each answer,
preferably in bold. Spelling and grammar count (one quarter of the total score for the
written assignment) so using a spell checker and/or a medical dictionary is advised.
Common grammatical errors such as those in capitalization, agreement of subject and
verb, improper use of affect and effect, principle and principal, or to, too or two, run on
sentences and free standing clauses without a verb should be avoided. Do not copy the
case history into the report. The report should be uploaded to Assignment
Submissions as an attachment; do not write it directly into the assignment page.
References and Citations References must be listed on a separate page at the end of the
report and follow the APA guidelines. Citations in the text are required, preferably at the
end of a sentence or paragraph. When you use APA style, the reference list should be in
alphabetical order. If you use the same reference for an entire paragraph then only one
citation at the end of the paragraph is required. For help with citations and references in
the APA style click on Academic Resources, launch the library link and then click on
Copyright and Citation in the blue bar at the top right. Choose the link to APA format.
This contains a template and detailed instructions for all things APA. Plagiarism is
another link in the drop down list. This section should also be reviewed to review the
rules of what must be cited.
