resulted from pilot error. Determine the primary and any secondary human factors contributions to the accident.

etc., when identifying an issue for your case analysis. As always, draw upon previous module knowledge to help
you conduct your analysis.

I. Summary
On February 12, 2009, a Bombardier DHC-8 operating as Colgan Air flight 3407, crashed while
on instrument approach to Buffalo-Niagara International airport, resulting in the deaths of 49
people onboard and one individual on the ground. While the National Transportation Safety
Board (NTSB) cited the pilot’s inappropriate response to the aircraft stall warning system, a
number of secondary factors, including fatigue, were also listed as contributing factors to the
crash (NTSB, 2010).
II. Problem
Crew fatigue has been citied over the years in a number of fatal aviation crashes in addition to
crashes involving other modes of transportation (Quan and Barger, 2015).
III. Significance of the Problem
Despite many attempts to reduce crew fatigue in aviation, fatal accidents still occur from the lack
of performance that results when aircrew are not at their peak performance. In order to ensure the
safety of the travelling public, the issue of poor crew performance due to a lack of rest needs to
be addressed. . .
IV. Development of Alternative Actions
Alternative Action 1. Require at least one crewmember with a higher rest time than currently
required by other flight crews.
Advantages. Since one crewmember will have a higher level of alertness, the likelihood
of fatigue induced accidents in air transportation will be reduced (Borghini, Astolfi, Vecchiato,
Mattia, and Babiloni, 2014).
Disadvantages. It may be difficult in rotating crews so there will always be one extended
rest crew member available for flight, thereby increasing the possibility of cancelled flights
(Weide, Ryan, and Ehrgott, 2010).
Alternative Action 2. Utilize some type of alert sustaining drug for crews on duty.
Advantages. Some studies seem to indicate that drugs such as Modafinil help to reduce
drowsiness without any detrimental side effects to the flight crew members (Estrada, Kelley,
Webb, and Athy, 2012).
Disadvantages. Not every individual reacts the same way to medication and further
testing needs to be done to ensure there are no detrimental side effects in crew members (Estrada
et al., 2012).
V. Recommendation
A change in the required number of hours of crew rest for all flight crew members for both large
and regional carriers should be incorporated by the FAA to ensure that pilot fatigue does not
become one of the many causes of fatigue induced error in aircraft accidents . . .
