One of the major ethical dilemmas fronting nurses today is the question of pro-life
choices, and specifically the issue of abortion. Over the past decade, there has been a wide
variation of attitudes and opinions regarding the approval or disapproval of an individual’s
pregnancy, and this has been such a problem because it comprises choices between life and
death. On the one hand, women may be prompted to deliberate abortion due to pregnancies
which occur from rape or even due to fetal and maternal mortality. On the other hand, every
human is eligible to the right to life irrespective of whether they have a disability or not.
Nurses and medical practitioners are frequently confronted with a choice of either to terminate
the life of the fetus and help the mother or endanger the life of the mother and save the fetus.
Deciding to let the fetus and endangering the life of the mother live would violate the nursing
ethical principal to preserve life while removing the fetus and letting the mother live would
break the law against ending the life of another human being. According to the Human
Fertilization and Embryology Act as amended in 1990, the law allows for the termination of
pregnancy if it has satisfactorily been found that if the child is born, it will suffer from mental
or physical abnormalities which can cause it to be handicapped (Kumari, Memon, Bhutta &
Jamelle, 2016).
The major ethical and legal differences in this dilemma are that allowing a pregnancy to
advance may cause serious problems and complications to the mother including both physical
and mental problems which can all put the life of the mother at risk. In some cases, it can lead
to the death of the mother. However, terminating the pregnancy may not only end the life of
the fetus but may also put the life of the mother in a precarious position (Shields et al., 2016).
Before deciding to proceed with any action under these circumstances, it is important or
nurses to go extensive consultative and collaborative actions to arrive at the best solution.
Another issue which can enable the amicable solution to this dilemma is ethical training to
both nurses and other medical professionals (Yang, Che, Hsieh & Wu, 2016). Finally,
practicing patient based care and ensuring cultural competent nursing care can be used to
arrive at the best solution for both the mother and her pregnancy.

This is a topic that can cause moral distress if you work in a
facility that is pro-choice but you are not in agreement with that
treatment on a young patient. To make matters difficult, what if
the young woman is having an abortion because she knows that
her child may have Down syndrome? A study found that students
in Turkey supported abortion if a woman had a child with a
mental disease or Down syndrome, with women being able to
receive said abortion past 10 weeks if the baby does have Down
syndrome (Yanikkerem et al., 2018). This number increased
greatly in women from Norway. I Can imagine how difficult it
would be to support patients’ decisions with which you do not
agree.

You need to describe a specific scenario, not a general ethicallegal dilemma that would cause the advanced practice nurse
moral distress. Be specific about the dilemma, just like you are
telling a story. Use the case studies in your textbook as
examples.

Hi Beverly,
I enjoyed reading your post. Abortion can be a very complex topic in healthcare
related to either religious perspective or feminism principle. Badruddin (2016) stated the
American Nurses Association require nurses to maintain their professional responsibilities
while providing nursing care to patients who had abortions. The author also expressed
healthcare professionals has an obligation to respect the autonomy and dignity of the pregnant
woman, the embryo and the justification for termination of pregnancy. I believe women has
the right to decide what they want to do with their body however I do believe counseling
should be offered in certain situations before abortion. Thank you.
