The measures from the list that can aid in the improvement of quality of patient outcome
are the smokers’ cessations and the weight assessment. The smokers are advised to quit and
cessation recommendations were recommended without any follow up. In attempt of ensuring
that the patient outcome improves, there should be acquisition of data that can be used for further
examinations and ensuring that the patients took the advice into practice. The physician practice
therefore seeks that the quality of patient outcome and in this case the smokers can be achieved
through following up on the patient till their recovery and complete quit of smoking so as to
ensure that the recovery outcomes are successful.
On the weight management aspect too, the measures can be improved by ensuring that
the medical facility does not only advice patients in the outpatient department but also be part of
the champions to help patients in the weight loos process. The facility is supposed to involve the
patients in the physical activities with some guidance of experts. By advising them to get into
physical activities and leaving it at that would not yield much in terms of patient outcome
because some may end up ignoring or overdoing the exercises and end up getting health risks
again.
On the smokers’ cessation, the data can be obtained from the hospital rehabilitation
Centre and the data weight assessment can be collected from the outpatient department records.
Plan-Do-Study-Act (PDSA) model is the best to apply in implementation of these measures
(Liebler et al 1998). On the smokers’ cessation, a proper plan on how to follow up about the
recovery of the patients should be drawn, the patients are received in the facility, the physician
carefully assesses their level of damages caused by the smoking and finally the situation is acted
on. The weight assessment is also similarly done as smokers’ cessation. There must be a plan on
how to deal with the weight loss issue systematically such that the patients are not affected
eventually. The assessment process is done and study is done on how best the patients can lose
weight and finally put them into practice. The health care sectors of the government are rising in
joining hands with HEDIS in implementing the health measures.
